What is Plastic and Reconstructive Surgery ?

The word plastic comes from the Greek word meaning "giving form." Plastic surgery encompasses both reconstructive surgery and aesthetic surgery, sometimes called cosmetic surgery.
Reconstructive surgery restores or improves physical function, minimizes disfigurement from accidents, disease, or birth defects. Aesthetic surgery, while not essential to physical health, can improve the appearance of body features and thereby enhance self image.

We are located at 5200 Park Avenue, just east of White Station Road in Memphis, Tennessee (See Map).

Hours

We are open for appointments from 8:30a.m. To 4:30 p.m., Monday through Friday.

Telephone Calls

Our telephone number is 901.765.4700. If you need to speak to your doctor, the Secretary will take your name, telephone number, and information on the nature of your call. In some case, the office nurse or other personal will be able to answer your questions. Your telephone call will be returned at the earliest possible moment. Always tell the Secretary immediately if you are making an emergency call.
After hours, our answering service will take your name and telephone number, and your call will be returned by our physician on call at the time. This service should be used only in the case of an emergency.

Scheduling Appointments

To minimize waiting time, we request that all patients make appointments. Every effort will be made to see you on time.
When making your appointment, please tell the Receptionist the nature of your visit so we can schedule sufficient time for you. Should you find it necessary to cancel an appointment, please notify us as far in advance as possible.
When you arrive for your appointment, please register at the Reception Desk. We can then have your records available for the doctor. If it is your first visit, you'll need to fill out a patient information form.

Hospitalization

While many minor surgical procedures can be performed in our clinic, some procedures require hospitalization. We perform surgery in most Memphis-Area Hospitals and health care facilities. If you have a preference, please inform your doctor.

Prescription Refills

Federal regulations determine the policy of prescription refills. Refill requests must be called in during office hours so your medical records can be reviewed by your doctor.

Medical Records

At your first visit, your medical record is created. Its contents are always treated with the strictest confidence, and released upon your written permission.

Financial Arrangements

Charges for aesthetic procedures are to be paid for in advance. We accept several major credit cards for such payments.
For reconstructive procedures (those covered by insurance) we will gladly file your insurance claims. However, your medical insurance coverage is a contract between you and your insurance company, and does not relieve you of the responsibility of payment. Unfortunately, we cannot accept responsibility for collecting, or negotiating, disputed insurance claims. It is important that you bring your insurance card and information with you on your first visit.
Some insurers now require pre-certification for surgery. It is to your benefit to familiarize yourself with the details of your insurance coverage.

Call us as far as possible in advance to make your appointment.

Arrive on time for your appointment.
When you are unable to keep an appointment, let us know as soon as possible.

Feel free to discuss our fees with us. We want you to understand our charges and be satisfied that they are reasonable and equitable.

Advise the Receptionist when you arrive of any change in you address, telephone number, insurance, etc.

Bring you insurance card and information with you, especially on the first visit.

Report to our staff or to your doctor any dissatisfaction you have about our services.
